We are happy to register new NHS patients within our catchment area.
How to register
To register with our practice please follow the link below to complete the online registration form and patient information details.
After you have completed and submitted the form you must EMAIL the surgery within 7 days with 2 forms of identification (ID) one being photographic and one with your name and address (post date within 3 months). Email address is: firstname.lastname@example.org
You will also be required to complete a health questionnaire and provide all vaccination history for any child you are registering under the age of 10.
Acceptable Identification; tenancy agreement, mortgage agreement, bank statement, council tax bill, driving licence, passport, birth certificate etc.
The reception staff are able to accept registration forms daily between the following hours:
Every morning: 10:00 – 14:30 (NOT CURRENTLY APPLICABLE DURING PANDEMIC SITUATIONS PLEASE DO ALL SUBMISSIONS ONLINE)
Every evening (excluding Wednesday): 16:00 – 18:00 (NOT CURRENTLY APPLICABLE DURING PANDEMIC SITUATIONS PLEASE DO ALL SUBMISSIONS ONLINE)
Until you have submitted your ID and completed the health questionnaire you will not be registered at the practice.

Once registered, make an appointment with our nursing team for a registration medical.
We invite all our new patients to see one of our practice nurses or our health care assistant. This appointment enables us to collect medical information before your medical records arrive. It is also the start of screening for other medical problems. Normally, you will need to complete a registration medical before seeing one of our GPs.
